If you have registered for a course with a Service-Learning component or are planning to take one, you will need to print and fill out the documents on this page.
The Service-Learning Contract serves to clarify your site supervisor's expectations for you during your service. Bring this form to your chosen site's volunteer orientation, making sure to fill out the 'course objectives' section in advance. Have your site supervisor sign the form and go over it with you, and then return it to your instructor.
Once you begin volunteering at your site, you will need to bring the "Quarterly Record of Service Hours" so that your site supervisor can vouch for your service.You will turn this into your professor at the end of the quarter.
This form must be signed and returned to your professor before a minor student can begin volunteering at a site for service-learning. (Only for those students 18 and under)
The Assumption of Risk is an understanding between the student and the College in which the student agrees to take responsibility for any damage that may occur during their service and the College is no longer liable. You must sign and return to your professor or coordinator in order to begin service.
You only need this form if you have chosen an opportunity that has not been pre-selected by the Service Learning Coordinator (a.k.a. an opportunity NOT listed on this website)
